BREAKING Fatal crash on takeoff—crew pronounced dead while #2

Tragic aviation incidents have struck in multiple locations, with the most devastating recent event occurring during landing rather than takeoff. At Miami International Airport, a catastrophic crash has claimed five lives, though the crew members survived this particular incident.

The Miami Cargo Jet Disaster

On September 6, 2026, an Amazon Prime Air cargo jet operated by 21 Air crashed while landing at Miami International Airport. The Boeing 767-300, arriving from San Juan, Puerto Rico, overran the runway and careened into vehicles on airport property.

The five people killed were all inside a white Ford van owned by Professional Ocean Service Corp., a company providing aircraft cleaning services. Seven cleaning crew members were in the van when the jet slammed into it; the five who perished were identified as:

  • Rolando Aleman Leon, 55
  • Yoel Rodriguez Naranjo, 53
  • Julio C. Pineda, 75
  • Carlos Acosta Fajardo, 53
  • Javierkys Reyes Quevedo, 47

Crew Members Survive

Despite the severity of the crash, the pilot and co-pilot were released from the hospital and are expected to recover. Witnesses reported seeing the pilot jump from the burning cockpit shortly after impact, escaping as flames engulfed the aircraft. Two other individuals remain in critical condition.

Investigation Underway

The National Transportation Safety Board (NTSB) has recovered both the flight data recorder and cockpit voice recorder for analysis. NTSB Chairwoman Jennifer Homendy described the crash scene as “utter devastation,” with debris scattered across a wide area.

Investigators are examining why the aircraft appeared to touch down far beyond the intended landing point, remaining airborne for an extended period before touching down. The lack of an Engineered Materials Arresting System (EMAS) at the runway’s end—designed to slow aircraft that overrun runways—will be a key focus of the investigation. Miami is among airports without this safety feature.

Other Recent Takeoff Incidents

While the Miami crash occurred on landing, other recent takeoff incidents have resulted in fatalities:

  • In Titusville, Florida, a pilot died after a small plane crashed shortly after takeoff from Space Coast Regional Airport, striking trees and power lines.
  • In Australia, a father-daughter duo died when their Beechcraft A36 Bonanza crashed into a field minutes after takeoff from a Brisbane airport.
  • In Indonesia, a cargo pilot was pronounced dead following a crash in North Kalimantan.

As authorities continue their investigations, the aviation community mourns the lives lost while searching for answers to prevent future tragedies.
